A Study on the Willingness To Pay for NHI among International Students in Korea - using Double-bounded Choice Contingent Valuation Method

Abstract
The purpose of this study was to elicit willingness to pay (WTP) for National Health Insurance (NHI) through the lens of international students in Korea. We also aimed to examine factors influencing WTP among international students. The research design for this study was double-bounded dichotomous choice (DBDC) contingent valuation. A cross-sectional online survey was conducted among a sample of international students at universities(n=5,333). Participating students completed an online questionnaire that included questions related to their socio-economic characteristics, health status and behaviour, medical insurance, and WTP. Outcomes of this study showed that the WTP to NHI among international students in Korea is 18,708 KRW(¡¾451 KRW). Factors influencing the WTP was gender, age, marital status, nationality, visa types, average monthly expenditure, self-rated health, living with a chronic illness, past experience in claiming and receiving reimbursement, and the number of monthly outpatient visits. Higher WTP was derived from students who are men, older, married, not Chinese, with visa types expected to stay longer in Korea, spending higher average monthly expenditure, reported lower self-rated health, living with a chronic illness, having experience in claiming and receiving reimbursement, and visited outpatient clinics more often than their counterparts. This study demonstrated that actual NHI contributions, effective March 2021, are about 2 times higher than the elicited WTP among international students. Government needs to reconsider the WTP of international students when it decides on the contribution rate for NHI.
